Abstract

Arachnoiditis is an inflammatory disorder of the arachnoid membrane of the brain that may lead to leptomeningeal adhesions, disturbances of cerebrospinal fluid dynamics and focal neurological deficits. This article reviews etiology and pathogenesis, with emphasis on clinico-neurological manifestations — headache, meningeal signs, cranial nerve palsies, seizures, cognitive and behavioral changes, and signs secondary to CSF circulation impairment. Modern diagnostic approaches (contrast MRI, CT myelography, CSF analysis), differential diagnosis and treatment principles (from conservative antiinflammatory and rehabilitative care to neurosurgical options) are discussed
